Need to be off sick ?

As your employer the Trust needs to know when you are off sick.

So do the following to let the team and the Trust know:

Hammersmith Hospital Contact:

• SpR holding bleep 9007
• who will inform your Consultant / On-call Consultant Anaesthetist
•  Email Enda (Department Office Manager)

Queen Charlotte's Hospital:

• SpR 1st on Bleep 9404
• who will inform the On-call Consultant Anaesthetist
• Email Enda (Department Office Manager)

ON YOUR RETURN TO WORK:

• Complete a ‘Self Cert’,
• Email your completed form to Enda (Department Office Manager)
• Self certification can be downloaded from this link on The Source (so inside the Trust only)

This is a legal requirement.

IMMEDIATELY:

on your return to work, complete a ‘self cert’ form
