On the whole Hammersmith is a receiver and not a sender of critically ill patients. Although we will be transferring patients for some forms of surgery and neurological care and support whilst critically ill.
As members of the NWLCCNetwork we use the standard recording forms and audit tools. ALL TRANSFERS must be recorded as the very act of transfer can lead to morbidity.
We have a slight variant on the Internal transfer form to the network standard on CICU as there is more space for infusions and an anaesthetic chart feel to the back side of the local form.
The INTER or between hospital transfer forms are held in theatres by ODP's and must be used for every outside of the building transfer of Level 1-3 that an anaesthetist is involved with.
